STEP 1: Register and Schedule Biometric Screening on My Wellness Portal

Register at My Wellness portal to start the scheduling process. My Wellness portal will help guide you through the process. Follow the instructions below to login with your user name and password.  

  •  User name for My Wellness portal:
    • Employees: Your user name is your employee ID.
    • Spouses: Your user name is the employee’s ID with an “S” added at the end (e.g. 12345S).
       
  • Password:  
    •  Your password is your date of birth (MMDDYYYY).
    •  After successfully logging in, you will be prompted to create a new password.
    •  Record your password and save. You will need this to log-in to take your health assessment or change your biometric screening appointment times.

While logged in to My Wellness portal, schedule your biometric screening. Click the “Schedule Your Screening” button to choose your appointment time and location. 

STEP 2: Complete health assessment, AUG. 1 - AUG. 25

Employees and spouses can take the personal health assessment on the My Wellness portal between Aug. 1 and Aug. 25.

  • If you are returning to the site to take the assessment, you will need to log in. Refer to your user name and password you saved during your My Wellness portal registration.
  • Click the “Take Health Assessment” button to complete assessment and follow these instructions

STEP 3: Complete Biometric Screening, AUG. 1 - AUG. 25, 2017

Visit your selected site and complete your biometric screening.

STEP 4: Enroll in the Wellness Plan via PeopleSoft in November

The Open Enrollment period is for all employees and covers your options for health insurance and the many other benefits you receive as a Samaritan employee. Don’t miss this last step, which is the final step you must take to qualify for the lowest premium in 2018.
