Alternate Log-in Instructions to Access the My Wellness Portal

If you’re having trouble logging into the My Wellness portal, or are not a current member, use these alternate log-in instructions.

Go to AssetHealth.com/SamaritanHealthPlan and follow the instructions below to set up your account and create a username and password.

  • Username
    • Employees: Your username is your 5-digit employee ID number.
    • Spouses/domestic partners: Your username is the employee’s 5-digit ID number with an “S” added at the end (e.g. 12345S).
  • Password
    • Your password is your date of birth (MMDDYYYY).
    • After successfully logging in, you will be prompted to create a new password.

Record your password and save. You will need this to log into the My Wellness portal, to take your health assessment, and to schedule or change your biometric screening appointment.
