Health Risk Assessment + Biometric Screening = Your Health Risk Score

The results of your health risk assessment and biometric screening add up to a risk score. This number will fall into one of four risk categories: Low, Borderline, Moderate or High.

The purpose of your score is to provide you with a snapshot of your current health, including a report that will help you see where you are doing well, and identify specific areas you can take real and meaningful steps to invest in your health, wellness and longevity.

There are two steps you need to take to receive a confidential health risk score and report. These steps will help you see how you are doing today and learn what you can do to improve your health risk score.

Step 1: Complete an online health risk assessment
The online health risk assessment is a questionnaire that asks you questions about your lifestyle, behaviors and medical history.

Step 2: Complete a biometric screening
The biometric screening results are based on a laboratory analysis of a blood sample (finger stick) taken from an onsite biometric screening as well as your body mass index (BMI) and blood pressure. These measures help identify future health risks.

Understanding your risks for certain diseases and conditions that can affect your future health can help you make health decisions that are right for you and your family.

Even healthy individuals have discovered areas of concern through a biometric screening. If your results do not indicate a problem, it verifies that your chosen lifestyle is working. By participating in the biometric screening, you will:

  • Find out what your health indicators or “numbers” are (blood pressure, cholesterol and glucose levels)
  • Identify potential health risks
  • Receive an informational handout with your personal results 
Any Samaritan employee and their spouse or domestic partner is eligible to participate.
To register for the biometric screening, current Samaritan Choice Plan members must access their secure member portal at MyHealthPlan.samhealth.org. If the employee or spouse is not a member of a Samaritan Choice plan, view the instructions for non-members. Although walk-ins will be accepted, those with scheduled appointments who check in on time will take priority.
For technical support with the biometric scheduler, please contact Interactive Health Member Services at 800-711-8656 and they will be able to assist you. They are open Monday through Friday from 5 a.m. to 2 p.m.
Interactive Health is the vendor who performs the onsite biometric screenings. They are currently the country’s leading provider of health management solutions. The company is backed by the National Committee for Quality Assurance (NCQA) with Performance Reporting and has earned exclusive endorsement by the American Hospital Association for workplace wellness solutions.
Walk-ins will be accepted only if time permits, without interfering with those participants who have scheduled appointment times. Walk-ins are not a guarantee. However, the vendor will make the best effort to accept walk-in appointments.

Participants who arrive earlier than their scheduled appointment time may have to wait to begin the registration step. Participants who arrive later than their scheduled appointment time may have to wait until there is an opening within the process.
No. Samaritan Health Services is funding the cost of this program and your participation is free. This is part of our commitment to creating a healthier community.

The biometric screening will take up to 20 minutes and includes:

  • Up to 17 minutes to check-in, conduct finger stick, run tests, take your blood pressure and height/weight 
  • 3 minutes to review results 
The biometric screening includes measurements for cholesterol (HDL and TC/HDL ratio), blood glucose, blood pressure and Body Mass Index (BMI).  Results will be reviewed during your screening appointment with a health professional.

The finger-stick method of blood draw is used to measure cholesterol and blood glucose. This is easy and fast.  The screener will “prick” your finger and place the blood sample into the Cholestech machine.

When arriving to your biometric screening follow these helpful tips to ensure the process goes smoothly:

  • Employees bring your employee ID badge
  • Spouse / domestic partners bring your Samaritan Choice Plans insurance ID card or provide the employee ID number for your spouse or domestic partner.
  • Wear sleeves that can be rolled up easily so the examiner can take your blood pressure measurement.
  • Drink plenty of water to keep hydrated.
  • Arrive at least 5 minutes prior to your scheduled appointment time to register and complete the Screening Consent Form.
No, fasting is not required for this screening. Non-fasting tests are accurate for measuring your cholesterol and blood glucose. If your test results are borderline or high, the staff at the screening will encourage you to see your physician for a comprehensive fasting blood test.
 The advice of the screening staff is a recommendation only and should not be interpreted as medical advice. You will be responsible for all charges not covered by your health plan.
 
Yes, all information is treated as confidential medical information. You allow Interactive Health, a third party, to receive this information by signing the Screening Consent Form. Your personal results will not be shared with your employer. Interactive Health administers the screenings to ensure confidentiality and is held to strict privacy and security laws.
You will need to sign a Screening Consent Form to participate in the screening. If you do not consent, you will not be allowed to participate. Interactive Health is required by law to receive employee consent prior to obtaining confidential medical information.
All personal identifying information (name, address, ID or insurance information) is stripped out of the data. Only the test results are shared in a data / report format. For example, it may show that out of 50 employees tested, 20 had total cholesterol levels higher than 250.
An online health risk assessment is a valuable tool that provides you with personalized information about your health risks and strengths. When you complete the online health risk assessment, you will see a green check mark displayed indicating completion.
Current Samaritan Choice Plan members can access the online health risk assessment from their secure member portal at MyHealthPlan.samhealth.org. If you are not a member of a Samaritan Choice plan, view the instructions for non-members. You can use your mobile phone or tablet to complete the assessment.
Taking the online health risk assessment is like giving yourself an annual health checkup. The report will help you focus on the things that can have the biggest impact on your health. You’ll also get suggestions on how to take action with online tools or other resources available to you.
Taking the online health risk assessment is quick and easy — it only takes about 15 minutes to answer questions about your health and health habits using the secure, online tool.
The health information needed and topics covered in the health assessment include: personal health history, health perception, medications, nutrition, portion size, exercise, weight, tobacco use, skin cancer prevention, immunizations, gender only, colonoscopy, oral health, vision, stress and life management, alcohol consumption, engagement and change, and Employee Assistance Program (EAP).
The questions will not be available until the Wellness Campaign events start.
Small changes in health habits can make a big difference in determining your health risk. When you take the online health risk assessment over time, you’ll see where you’re doing better and areas that may need improvement. In quick and easy terms, you will have an idea of where you need to take action.
Once you complete your online health risk assessment and the biometric screening you will be provided a personalized health risk report. This report will help you take real and meaningful steps to invest in your health, wellness, and longevity. Samaritan Health Plans encourages you to see your doctor if there is anything out of range.
The My Wellness portal has a wealth of tools and resources that can be used throughout the year.  Log on through MyHealthPlan.samhealth.org at any time to track your health and activity, review your online health risk assessment and read health content.

No. The biometric screening and health assessment is FREE to any employee and their spouse or domestic partner.

No, the incentive is only available for employees who are Samaritan Choice Wellness Plan subscribers (the employee who has the health plan premium withheld from their paycheck).
Samaritan Health Services will receive only combined statistics stripped of any identifying information. This group data will help Samaritan evaluate current health programs and look for new ways to enhance our wellness benefits in future years. 
The online health risk assessment and onsite biometric screening will be managed by assigned vendors. No personally identifiable information will be shared with Samaritan Health Services as your employer. Remember, it’s against the law for your employer to access your personal health information. Your personal health information will be handled by your health plan, Samaritan Choice Plans.
The vendors Samaritan has selected to conduct the biometric screenings and provide the online health risk assessment use a secure website platform and store the health risk assessment and biometric screening data in compliance with all current federal regulations for privacy, security, and electronic data interchange (EDI). This includes but is not limited to HIPAA, the Americans with Disabilities Act (ADA), the Genetic Information Nondiscrimination Act (GINA), as well as all state requirements that give additional protection to sensitive protected health information (PHI).
The Acknowledgement Agreement Form contains information regarding the Wellness Program for the Samaritan Choice Wellness plan and will be displayed in PeopleSoft when you enroll.
