For 2021, Samaritan employees who are Choice Wellness Plan subscribers (the employee who has the health plan premium withheld from their paycheck) can receive $150 by earning a total of 20 points for completing a variety of health-related activities. Options include a routine physical exam, preventive screening (like the COVID-19 vaccination series), dental exam, flu shot, online Self Help Works courses and financial wellness events.

Preventive routine physical exam Your points will be automatically uploaded to the My Wellness portal within 90 days after your exam. 8 points
Preventive dental exam Self-report your dental exam through the My Wellness portal. Click the My Wellness Incentives tile, then select View My Incentives Programs and go to the 2021 Healthy Habits Incentive. 5 points
Flu shot Self-report your flu shot through the My Wellness portal. Click the My Wellness Incentives tile, then select View My Incentives Programs and go to the 2021 Healthy Habits Incentive. If you receive your flu shot from a provider, your points will be automatically uploaded to the My Wellness portal within 90 days after your flu shot visit.  2 points
Preventive screening  Complete one of the following: mammogram, colorectal screening, prostate exam or pap smear. Your points will be automatically uploaded to the My Wellness portal within 90 days after your visit. 8 points; maximum of 8 points allowed, even if you complete multiple screenings
Online Self Help Works course
Complete an online course through the My Wellness portal. Click the Self Help Works tile, then select a course. Eight courses are available. 5 points/course; maximum of 15 points allowed
COVID-19 vaccination series Complete the COVID-19 vaccination series. Your points will be automatically uploaded to the My Wellness portal within 90 days after the COVID-19 vaccination series is completed.   8 points
Financial Wellness events Attend any of the following SHS sponsored Financial Wellness events. Contact your local Samaritan Human Resources department regarding available event dates and times. Points for these events will be automatically uploaded to the My Wellness portal on a quarterly basis.
Principal:
One-on-one Retire Secure meeting.
Complete a MyVirtualCoach experience within your Principal account.
Money Management International (MMI):
One-on-one or group education meeting.
NW Mutual/Dragoo Financial: 
One-on-one or group education meeting.

2 points/event; maximum of 10 points allowed
(points will not be awarded for duplicate events)

Eligibility Rules

  • The incentive period begins on Jan. 1, 2021 and ends on Dec. 31, 2021.

  • Employee must be enrolled as the subscriber on the Samaritan Choice Wellness Plan as of Dec. 31, 2021 in order to be eligible to participate in the 2021 incentive.

  • All points earned will be tracked online through the My Wellness portal, which can be accessed through My Health Plan.

  • Points will automatically be uploaded into the My Wellness portal upon receiving a claim from your provider after a preventive routine physical exam or preventive screening. This will occur within 90 days of your exam or visit.

  • Subscriber on the Samaritan Choice Wellness Plan must complete at least 20 total points out of the possible 56 to qualify for the incentive for 2021. The $150 incentive will be paid out within the first two quarters of 2022.

  • Service dates for eligible activities must be rendered within the incentive period and occur during time of employment with Samaritan Health Services.

  • Services must be provided within the incentive period while the employee is enrolled as a subscriber on the Samaritan Choice Wellness Plan.

  • Employee must be an active employee at time of pay out in 2022.

  • All self-reporting activities including dental exam and flu shot must be entered no later than Jan. 15, 2022. All activities recorded in January 2022 must have been completed by Dec. 31, 2021 to be eligible for the 2021 incentive.

Yes. For the SHS designated Financial Wellness Events, the live or recorded webinar with Principal has been replaced with MyVirtualCoach. Points can be earned when you complete a MyVirtualCoach experience with your Principal account. This financial wellness event is worth 2 points.

Track your points on the My Wellness portal (accessed from My Health Plan). Once on the portal, go to the “My Wellness Incentives” tile, click “View My Incentives Programs” then “View Program” to track your points and see your program completion status.

Points are automatically uploaded within 90 days after receiving the claim from your provider for a preventive routine physical exam or preventive screening. You must have at least 20 total points to qualify for the $150 Healthy Habits Incentive for 2021, which will be paid out within the first two quarters of 2022. 

Go to My Health Plan

You are able to self-report points for your preventive dental exam and flu shot by logging into the My Wellness portal (accessed from My Health Plan) and going to the My Wellness Incentives tile. Click on View My Incentives Programs and access the 2021 Healthy Habits Incentive program. Click on the plus sign (+) for the preventive dental exam or flu shot and select Sign Here to report the completion.

Your 2021 Healthy Habits Incentive will be taxable and paid out within the first two quarters of 2022. You must have at least 20 total points and be an active employee at time of pay out in 2022 to qualify for the $150 Healthy Habits Incentive.
