Although you cannot earn more points for your 2018 Healthy Habits Incentive, you can continue to track your 2018 point status in the My Wellness portal. Do not worry if points from your routine physical exam or preventive screening have not populated yet. Claims data for those point options can take up to 90 days to reflect in the Incentive Tracker. 

All activities must have been completed by Dec. 31, 2018, to be eligible for the 2018 Healthy Habits Incentive. All self-reporting activities, dental visits and flu shots, must have been entered into the Incentive Tracker no later than Jan. 15, 2019.

2018 Eligibility Rules:

  • Employee must be enrolled as the Subscriber on Samaritan Choice Wellness Plan as of Dec. 31, 2018 in order to be eligible to participate in the 2018 incentive.
  • All points earned will be tracked online through the member portal, MyHealthPlan.samhealth.org – points.
  • Data will automatically be uploaded into the My Wellness incentive tool within 90 days after a routine physical visit or preventive screenings upon receiving claim from your provider.
  • Subscriber on the Samaritan Choice Wellness Plan must complete at least 20 total points out of the possible 39 to qualify for the incentive during each incentive year, in this case 2018, of which $150 to be paid out within the first two quarters of 2019.
  • Service dates for eligible activities must be rendered within the incentive period and occur during time of employment with Samaritan Health Services.
  • Employee must be an active employee at time of pay out in 2019.
