Complete the Stress Free Challenge to Earn Two Points For Your $150 Wellness Incentive!

Take the challenge from Oct. 16 - Nov. 26.

At Samaritan Health Plans, we do more than take care of you when you are sick—we work to keep you healthy. One way we do this is to provide you with extra incentives and support for preventive care and well-being. 

For 2017, Samaritan employees who are Choice Wellness Plan subscribers (the employee who has the health plan premium withheld from their paycheck) can receive $150 by earning points for completing a variety of health-related activities, such as an annual physical exam, a dental preventive exam, and a flu shot. Additional opportunities will be provided for interactive online courses and health challenges that will allow you to join up or compete with colleagues for fun and good health. 

Subscribers who earn 20 points during 2017 will qualify for the $150 incentive, which will be taxable and paid out in Q1 2018.

Track and Manage Your Incentives Online

Points will be tracked online through your member portal, You must have a member account to access the incentive tracker. All communications and notifications about your incentives and their options will be sent to the email address that you provide when setting up your member account.

Below are the point options offered for 2017.

Preventive Routine Physical Exam Your claim data will be automatically uploaded to the online incentive tracker within 90 days after your exam. 8 points
Preventive Dental Exam You will report your dental visit. See instructions within the online incentive tracker. 5 points
Flu Shot You will report your flu shot. See instructions within the online incentive tracker. 2 points
Preventive Screening  Complete one of the following: mammogram, colorectal screening, prostate exam, pap smear. Your claim data will be automatically uploaded to the online incentive tracker within 90 days after your visit. 8 points
Online Knowledge Management Course
You will need to complete an online course with a passing test rate of 70% or higher. Ten course options available. See instructions within the online incentive tracker. 5 points/course; maximum of 10 points allowed
Online Health Challenge Look for email communications* to be sent to you regarding when challenges will be running and how to participate. Three challenges will be offered in 2017. 2 points/challenge; maximum of 6 points allowed
 Eligibility Rules:
  • Employee must be enrolled as the Subscriber on Samaritan Choice Wellness Plan as of 12/31/2017 in order to be eligible to participate in the 2017 incentive.
  • All points earned will be tracked online through the member portal, – points.
  • Data will automatically be uploaded into the Asset Health incentive tool within 90 days after a routine physical visit or preventative screenings upon receiving claim from your provider.
  • Subscriber on the Samaritan Choice Wellness Plan must complete at least 20 total points out of the possible 39 to qualify for the incentive during each incentive year, in this case 2017, of which $150 to be paid out within the first two quarters of 2018.
  • Service dates for eligible activities must be rendered within the incentive period and occur during time of employment with Samaritan Health Services.
  • Employee must be an active employee at time of pay out in 2018.
  • Quarterly health challenge activities must be reported during time of each  and points are not applied or eligible post challenge dates.
  • All self-reporting activities including dental visit, flu shot or knowledge courses must be entered no later than by January 15th, 2018

Here’s How the Stress-Free Challenge Works:

Each week of the Challenge, from Oct. 16 to Nov. 26, you will be sent helpful tips and suggestions for stress-reducing activities. You will need to record at least 55 activities during the challenge to earn two points for your $150 Wellness Incentive. Dec. 6 is the final date that activities can be recorded.

How to Record your Challenge Activities

  • Go to on your desktop or mobile device
  • Select the link to the $150 Wellness Plan Incentive
  • Select the Stress-Free Challenge button to open the Challenge and begin recording your activities.
